At Choices Australia Support Services, we provide compassionate, trauma-informed and person-centred supports for individuals living with psychosocial disability, complex mental health, dual diagnosis and other disabilities.

We understand that no two people are the same. Our supports are flexible, individualised and designed to empower participants to build stability, independence, confidence and meaningful connection within their communities.

Our services include:

  • Our Support Coordinators work alongside participants to help them understand and implement their NDIS plans, connect with the right services, and build the confidence to navigate supports independently over time.

    We assist participants to:

    • Understand their NDIS funding and budgets

    • Connect with providers and community services

    • Coordinate complex care teams

    • Navigate housing, health and psychosocial supports

    • Build capacity and independence

    • Prepare for plan reviews and reporting

    • Strengthen informal and community supports

    We have extensive experience supporting participants with complex mental health, trauma histories, dual diagnosis, homelessness risk, forensic backgrounds and high service complexity.

  • We provide practical and capacity-building support to assist participants in daily life and community participation.

    Supports may include:

    • Community participation

    • Attending appointments

    • Shopping and errands

    • Building social confidence

    • Transport support

    • Daily living skill development

    • Meal preparation assistance

    • Routine and structure support

    • Support to engage in hobbies, study or employment pathways

    Our focus is always on building independence, confidence and meaningful engagement within the community.

  • Our Recovery Coaches support participants living with psychosocial disability to build resilience, increase independence and work toward meaningful recovery goals.

    We take a strengths-based and recovery-oriented approach that focuses on:

    • Building daily living skills

    • Emotional wellbeing and self-management

    • Increasing confidence and social connection

    • Developing routines and structure

    • Accessing mental health and community supports

    • Goal setting and long-term recovery planning

    • Reducing isolation and improving quality of life

    We work collaboratively with participants, families, clinicians and community supports to create sustainable and holistic recovery pathways.

  • Our STR supports provide participants and families with safe, supportive and restorative short-term care options.

    STR may include:

    • Community-based respite

    • Skill-building opportunities

    • Social and recreational activities

    • Increased independence

    • Support in a safe and welcoming environment

    • Opportunities to build confidence outside of the home

  • Our team has extensive experience supporting individuals living with:

    • Severe and persistent mental illness

    • Psychosocial disability

    • Dual diagnosis

    • Trauma-related presentations

    • Emotional regulation challenges

    • Complex support needs

    • Co-occurring disabilities

    • Substance use challenges

    We understand the importance of consistency, trust, therapeutic rapport and collaborative care when supporting people with complex presentations.

    We work closely alongside clinicians, hospitals, inpatient units, allied health professionals and community services to support safe and successful transitions back into the community.

  • We support participants transitioning from acute mental health settings, inpatient units and other supported environments back into the community.

    Our team assists with:

    • Coordinating services and supports

    • Establishing safe routines

    • Reducing risk of readmission

    • Connecting participants with community services

    • Supporting housing and tenancy stability

    • Building long-term psychosocial supports

    • Strengthening participant confidence and independence
