Submit A Referral

At Choices Australia Support Services, we welcome referrals for individuals living with psychosocial disability, complex mental health, dual diagnosis and other disability support needs. Our team carefully reviews all referrals to ensure we are the right fit for the participant’s individual needs, goals and level of support required.

We accept referrals from:

• Participants and families

• Support Coordinators

• Psychosocial Recovery Coaches

• Hospitals and inpatient units

• Allied health professionals

• Community organisations

• Mental health services

• Plan managers and other providers
